GENERAL SUMMARY/OVERVIEW STATEMENT
The Adult Medicine Case Manager is responsible for screening adult patients for depression and providing case management services to those who screen positive. In addition, the case manager provides services to other Adult Medicine and health center patients who require assistance, followup, or referral.
Services provided by the case manager to clients will include a complete assessment of behavioral risks, linking clients to needed health and social services, client advocacy and health education. The case manager will be responsible for all data collection for the Mind and Body project, as well as managing the project database and drafting reports.
The Adult Medicine Case Manager works under the supervision of the Nurse Manager and with the clinical supervision of the Director of Mental Health and Substance Abuse or a clinician she designates.
PRINCIPAL DUTIES AND RESPONSIBILITIES
- Provide routine mental health screening for Latinas seen in Adult Medicine, using the Mind and Body Project’s Depression and Behavioral Risk Screening and Assessment (DBRSA) Tool.
- For clients who screen positive with the DBRSA screening tool, develops an individual service plan and provides short term case management, referrals and tracking according to case management protocols. Appropriately documents case management activities.
- Provides patient education on depression, substance use risks, domestic violence and the link between mental health and chronic disease for patients being screened.
- Refers patients being screened to services per health center protocols, including stress-reduction groups and/or other non-traditional stress reduction activities. For patients referred by their provider for mental health services, assists patient in accessing service on or off site.
- Provides appropriate education, counseling and services to women and families, refers clients as needed, coordinates a case/service plain and follow-up of client progress and works with a multi-disciplining clinical team to coordinate care.
- Collects data for the Mind and Body project, manages the database and drafts project reports
- Provides case management services to patients referred by Adult Medicine providers, including information on accessing resources and services, followup to ensure continuity of care, and intake assessment to evaluate needs within a full range of case management services.
- Attends project and departmental meetings and relevant community meetings.
- Works collaboratively with clinical providers and communicates with all team members about client status.
- Provides other services and carries out other duties as required.
QUALIFICATIONS
1. Bachelor’s degree with a focus on social services, or equivalent experience.
2. Twelve months or more experience in social service, health care, or community work.
3. Bilingual in Spanish and English required.

2. Good client interaction skills.
3. Ability to communicate well both orally and in writing.
4. Good planning and organizational skills.
5. Ability to exercise good judgment and take initiative.
